A couple weeks before your first visit, all new patients receive a “new patient info package” from our office. Please read the information provided and bring your completed paperwork to the consultation appointment.
If you received the package but misplaced the forms, feel free to download your paperwork by clicking on the appropriate file below. These files are our office’s medical history forms for both adults and children. Please bring this form filled out to your orthodontic consultation at our offices in Ocala and Gainesville.

"Dr. Montini is a warm, personable, highly trained and skilled orthodontist. I have seen truly phenomenal changes he has made in restoring proper bite in even the most difficult of cases. He not only restores the smile nature intended, but improves eating (chewing) and thus digestion, nutritional uptake, and overall health. Dr. Montini is great with kids, too!"— Amy G.
